This implements the feature request in issue #89.

Changes:

• Adds a new setting to always overwrite a file in place on refresh, rather than creating a temp file.
• Takes into account the setting "Always show diff prompt", which cancels out the new setting
• Alters the 'import file' context menu command so that it actually imports or refreshes the file that was right-clicked, rather than the open editor
